TNF\u03b1 bindet an spezifische Rezeptoren, die das Signal f\u00fcr den Entz\u00fcndungsprozess ausl\u00f6sen.<\/p>\n<p>Im Falle einer chronischen Entz\u00fcndung produziert das Immunsystem kontinuierlich TNF\u03b1, sodass die Entz\u00fcndung nicht aufh\u00f6rt. TNF\u03b1-Blocker unterbrechen diesen Kreislauf, indem sie an den Botenstoff TNF\u03b1 binden. Hierdurch kann das Molek\u00fcl nicht mehr an den Rezeptor binden, sodass die Signalkaskade des Entz\u00fcndungsgeschehens unterbrochen wird. Der Talspiegel (Trough Level, TL) ist definiert als die Arzneimittelkonzentration im Blut, die unmittelbar vor der n\u00e4chsten Infusion gemessen wird.<\/p>\n<p>Dar\u00fcber hinaus hat die Immunogenit\u00e4t einen Einfluss auf die Wirksamkeit des Medikaments. Sogenannte Anti-Arzneimittel-Antik\u00f6rper (Anti-Drug-Antibodies, ADA) binden an das Medikament und k\u00f6nnen zu einer Abnahme der Medikamentenverf\u00fcgbarkeit im Stoffwechsel sowie zu allergischen Reaktionen f\u00fchren. Er ist ein Ergebnis der Studie von Nils Vande Casteele et al. (KU Leuven), in der die Wirkung des Medikamentenmonitorings auf das Therapieergebnis mit TNF\u03b1 untersucht wurde.<\/p>\n<p>Die Studie zeigt die positive Wirkung von TDM auf die Therapieoptimierung und die Senkung der Behandlungskosten. Dar\u00fcber hinaus weist die Studie darauf hin, dass der Test auf Anti-Arzneimittel-Antik\u00f6rper nicht standardm\u00e4\u00dfig durchgef\u00fchrt werden muss, sondern nur bei Patienten mit einem nicht nachweisbaren Talspiegel von Infliximab.
